Case Manager Job Opportunity

We are seeking a skilled and compassionate Case Manager to join our team. As a Mental Health Specialist, you will provide case management and counseling services to individuals with mental health conditions, dementia, and support for their caregivers.

Key Responsibilities:

  • Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualized care plans and interventions.
  • Provide case management, counseling, and care coordination for clients with mental health conditions.
  • Prior experience or knowledge in elderly mental health will be an advantage.
  • Plan, organize, and implement community outreach, programs, and group work.
  • Collaborate with community partners and stakeholders to support clients with complex health and social needs.
  • Deliver talks to raise awareness of mental health and dementia.
  • Support and manage volunteers who are befrienders to clients with mental health needs.
  • To undertake any duties as assigned by the Head, Community Mental Health and the Executive Director.

Requirements:

  • Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
  • Experience in case management, group work, and community work will be an advantage.
  • Strong interpersonal and communication skills.
  • Able to work independently and collaboratively within a team.
  • Good administrative and computer skills with proficiency in MS Office.

Qualifications:

  • Grad Diploma/ Degree in Social Work.
