Program Lead



About the Role:

  • We are seeking a skilled professional to lead our Case Management and Counselling Services.
  • The successful candidate will provide case management support and counselling services to residents and their families.

Key Responsibilities:

  1. To plan, review and conduct rehabilitative programmes for residents.
  2. Preparing, maintaining case files, preparing Individualised Service Plans (ISPs), progress and closure reports.
  3. Referring residents and their families to appropriate agencies for further intervention.
  4. Analysing trends and emerging needs of counselling and case management services.
  5. Directing and overseeing the planning, development and implementation of case management services and projects.
  6. Developing and implementing outreach and communication plans for services.
  7. Providing supervision, guidance and advice to team members.
  8. Conducting regular meetings with team members regarding service delivery and standards.
  9. Ensuring that the programme meets the mission, objectives and procedures of case management services.

Candidate Requirements:

  • A Degree in Social Work, Psychology or Sociology from recognised universities.
  • Registered as an Accredited Social Worker or Social Service Practitioner will be an advantage.
  • A minimum 5 years of relevant social work experience in the field, preferably working with offenders, and with at least 2 years of supervisory experience.

Working Hours and Location:

5 days work week, Monday to Friday - 9am to 6pm. Halfway House location: catered to Male residents.
