  • Wound Care Specialist


Job Description:

The role of a Wound Care Specialist is to provide expert care and support to patients with complex wounds. This involves working closely with clinical teams to assess, plan, and deliver comprehensive care that meets the individual needs of each patient.


Key Responsibilities:
  1. Assess patients' wounds and develop personalized care plans to promote healing and prevent further complications.
  2. Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless transitions between hospital, outpatient, and community settings.
  3. Provide education and training to patients, families, and healthcare staff on wound management, prevention, and treatment.
  4. Conduct thorough assessments of patients' medical history, current health status, and social circumstances to identify potential barriers to effective care.
  5. Develop and implement strategies to address these barriers and improve patient outcomes.
  6. Work closely with other healthcare professionals to integrate wound care into broader treatment plans and ensure continuity of care.
Requirements:

Education: Degree in Nursing or State Registered Nurse certificate

Certifications: Advanced Nursing Diploma or equivalent preferred for Senior Staff Nurse position; WOCP/WOCN certification an added advantage

Experience: Minimum 2 years in clinical practice as Wound Care Nurse/Wound Care Specialist Nurse; experience in managing complex wounds an added advantage

Skills: Good interpersonal and communication skills; ability to work effectively in a team environment
