Employment Transition Specialist

We are seeking an Employment Transition Specialist to provide case management services, enabling individuals with disabilities to transition into employment. As a key member of our team, you will be responsible for conducting outreach, collaborating with stakeholders, and managing transition programs.

Job Responsibilities:
  • Manage cases and develop support strategies to secure and retain employment for clients
  • Assist clients in leveraging their support networks, including co-workers, caregivers, and the community
  • Impart social and soft skills to facilitate client integration and performance at work
  • Engage employers to promote inclusive hiring and market our programs and services
  • Collaborate with government agencies, hospitals, VWOs, and relevant organizations to facilitate employment transitions
  • Conduct outreach to hospitals and agencies to raise awareness of programs
  • Address stakeholder concerns and foster mutually beneficial relationships
  • Create collaterals and publicity materials for programs
Requirements:
  • Diploma or Degree in Social Work, Psychology, Healthcare, or related fields
  • Experience in social services, healthcare, or public services preferred; fresh graduates welcome
  • Ability to meet tight deadlines and follow up diligently
  • Team player with adaptability and versatility
  • Strong problem-solving, communication, and interpersonal skills
  • Proficiency in languages beyond English is an advantage
  • Proficient in Outlook, MS Word, Excel, PowerPoint
