Job Title: Clinical Instructor

Job Description:

We are seeking a highly skilled and experienced clinical instructor to join our team. As a clinical instructor, you will be responsible for implementing patient care in collaboration with the multidisciplinary teams (MDT) and according to assessed needs and health condition(s).

You will facilitate admissions, transfers, and discharges, ensuring discharge planning is done timely upon patient admission and appropriate to their condition.

The successful candidate will follow up and evaluate patients' progress according to planned care and readiness for discharge.

They will also collaborate with the MDT to deliver evidence-based care to patients to achieve desired care outcomes.

A key aspect of this role is monitoring patients for changes in condition, recognizing early signs of clinical deterioration using appropriate physiological assessments and escalating as required to enable timely implementation of interventions.

Additionally, the successful candidate will participate and contribute actively in MDT rounds, execute physicians' treatment and investigation orders promptly.

They will prepare and assist physicians with bedside patient procedures and treatments.

The clinical instructor will communicate and discuss patients' progress and response to treatment in collaboration with the MDT.

Demonstrating clinical skills, knowledge, critical thinking, and reasoning in the delivery of patient care is essential for this role.

The ideal candidate will assist the nursing team in problem-solving if there are any patient, clinical, or operational issues and seek appropriate assistance or escalate to senior nurse managers.

Delivering patient and/or family education on disease progression, prevention, and rehabilitation and assessing understanding of the condition to enable participation in self-health care management is also a key responsibility.

Required Skills and Qualifications:

The successful candidate will have a minimum degree in nursing and at least 5 years of relevant permanent inpatient experience and senior nursing experience, preferably in gerontology/palliative care with an advanced/specialist diploma in geriatric/community nursing and long-term care settings.
