Support Practitioner

We are seeking a skilled and compassionate Care Therapist to provide personalized support services to patients in their homes. The ideal candidate will have excellent interpersonal skills, be able to work independently, and demonstrate empathy towards individuals with diverse backgrounds and needs.
Key Responsibilities:- Deliver home-based care services that cater to the unique requirements of each patient
- Collaborate with healthcare professionals to ensure seamless care coordination and compliance with industry standards
- Provide education and training to family members and caregivers on various caregiving techniques and strategies to enhance patient outcomes
- Hold a minimum GCE 'O' level, Nitec or Diploma qualification
- Prior experience in healthcare at home is preferred but not mandatory
- Exhibit strong observational and problem-solving skills, with the ability to adapt to changing situations
- Demonstrate exceptional communication skills, both written and verbal
- Be passionate about delivering high-quality patient care and committed to continuous professional development
Ideal candidates will possess proficiency in Microsoft Office, experience in the healthcare industry, and excellent time management skills. A detail-oriented approach, coupled with the ability to prioritize tasks effectively, is essential for this role.
