Support Practitioner

We are looking for a Support Practitioner to join our team. As a Support Practitioner, you will play a vital role in providing targeted assistance to children and families enrolled onto our programme via a home visitation model.

Your key responsibilities will include:

  • Building child development knowledge and parenting skills through comprehensive assessments and case plans
  • Enhancing parent-child relationships and supporting parents in enabling their children to meet age-appropriate developmental milestones
  • Ensuring child safety and well-being, including supporting key life transitions such as primary school transition support

To be successful in this role, you will need:

  • A degree in Social Science or equivalent, preferably with experience in working with families and undertaking case management
  • The ability to adapt to changes and the rigours of a home visitation model
  • The willingness to conduct some home visits at night and converse in basic Mandarin to cater to Mandarin-speaking clients
