Job Title: Social Service Assistant
This role offers the opportunity to work in a supportive environment and contribute to the well-being of individuals and communities.
The ideal candidate will possess excellent communication skills, be empathetic and able to work effectively with diverse groups. Experience in social services or a related field is preferred but not essential.
Key Responsibilities:
- Assist in case management, including screening, follow-up and post-discharge referrals.
- Provide information on long-term care schemes to patients and their families.
- Conduct home visits as instructed by senior staff members.
- Facilitate means testing for inpatients and dialysis patients.
- Assist with admission assessments and discharge planning.
- Engage in continuous learning through case discussions, supervision and training.
- Prepare support groups and assist with administrative tasks.
- Ensure proper documentation of case files and referrals.
Required Skills and Qualifications:
- Diploma in Social Work or WSQ Diploma in Social Services.
- Fresh graduates are welcome.
- Minimum 4 years of social service experience.
- Proficient in MS Office.
- Good communication and written skills.
