Overview of the Role:

This is a specialist position that involves providing high-quality care and support to individuals with mental illnesses and their caregivers. The successful candidate will be an experienced and passionate professional in the field of mental health, who can work effectively with healthcare professionals, community partners, and families.

Key Responsibilities:

  • Care Needs Assessment: Conduct comprehensive assessments of clients' and caregivers' needs to develop individualized care plans and interventions.
  • Collaboration and Partnership: Work closely with healthcare professionals, community partners, and families to provide holistic and person-centered care.
  • Supervision and Guidance: Provide supervision and guidance to junior staff in managing casework and support services.
  • Programme Development and Delivery: Manage and coordinate post-discharge programmes in partnership with tertiary healthcare institutions, including monitoring clients' progress and programme outcomes.
  • Mental Health Interventions: Deliver psychotherapy and conduct mental health talks for clients and caregivers.
  • Community Engagement: Build and maintain strong networks with stakeholders and community partners to address service gaps and improve care accessibility.
  • Team Supervision: Supervise the team through individual and group supervision sessions.
  • Professional Development: Promote ongoing training and professional development of team members, aligned with AIC Mental Health Competency Framework.
  • Group Work Sessions: Plan, facilitate, and evaluate group work sessions for clients and caregivers.

Requirements:

  • Experience: 3-5 years of relevant experience in mental health and dementia care, with demonstrated competencies in case management, group work, programme development, and community engagement.
  • Qualifications: Graduate Diploma, Bachelor's or Master's degree in Social Work, or Nursing or Relevant Allied Health Qualifications.
  • Competencies: Strong knowledge of case management practices for individuals with mental illnesses and dementia, familiarity with current trends and developments in the health and social care sectors, effective team player with excellent interpersonal and communication skills, and bilingual abilities and knowledge of dialects.
