Care Coordinator
We are seeking a skilled and compassionate professional to provide case management and counseling services to individuals with mental health conditions, dementia, and support for their caregivers.
Key Responsibilities:
- Conduct client and caregiver assessments using the Bio-Psycho-Social-Spiritual (BPSS) model to develop and implement individualised care plans and interventions.
- Provide case management, counseling, and care coordination for clients with mental health conditions.
- Prior experience or knowledge in elderly mental health will be an advantage.
- Plan, organise and implement community outreach, programmes and group work.
- Collaborate with community partners and stakeholders to support clients with complex health and social needs.
- Delve into talks to raise awareness of mental health and dementia.
- Spearhead and manage volunteers who are befrienders to clients with mental health needs.
- To undertake any duties as assigned by the Head, Community Mental Health.
Requirements:
- Experience and/or knowledge in working with clients on Mental Health challenges and Person-Centred Care and recovery will be an advantage.
- Experience in case management, group work and community work will be an advantage.
- Strong interpersonal and communication skills.
- Able to work independently and collaboratively within a team.
- Good administrative and computer skills with proficiency in MS Office
Qualifications:
- Grad Diploma/ Degree in Social Work
