Job Opportunity

The Singapore Red Cross Society is seeking a skilled professional to support individuals with moderate to severe physical and intellectual disabilities.

This is an exciting opportunity to make a meaningful difference in the lives of clients and their families/caregivers.

  • Conduct initial screenings and assessments to determine the level of complexity, key risks, and protective factors of each case.
  • Establish rapport and build professional relationships with clients and caregivers.
  • Develop and implement individualized care plans that address client needs and goals.
  • Promptly update and document client information in a comprehensive log sheet.
  • Maintain accurate records and produce timely reports as required by departmental policies and procedures.
  • Cover for manager during long leaves.
  • Handle collection of programme fees and transport fees.
  • Prepare periodic reviews for statutory submissions and audits.
  • Maintain various social work reports, including monthly data updates.
Requirements:
  • Recognized degree or postgraduate diploma in social work.
  • At least 3 years of working experience in the social service sector and healthcare sector.
  • Good knowledge of government means testing framework.
  • Excellent case management skills.

  • Works effectively as an individual and as part of a trans-disciplinary team.
  • Self-initiative and resourceful.
