We are seeking a skilled and compassionate staff nurse to provide exceptional care to homecare clients.

Job Responsibilities

  • Client Care Management:
    • Conduct comprehensive assessments of new referrals to determine healthcare needs, developing individualized care plans accordingly.
    • Perform basic health screenings, monitoring client health status and making necessary referrals.
    • Administer self-management plans to empower clients and their families in managing health conditions.
  • Homecare Client Management:
    • Assist in planning and facilitating homecare services for clients, working closely with the manager and team to achieve targets and manage outcomes.
    • Maintain accurate documentation of client progress and data collation, preparing monthly statistics and reports as required.
  • Emergency Response:
    • Participate in medical emergencies or crisis situations as appropriate, monitoring early signs of client condition deterioration.

Requirements

  • A registered nurse with a diploma in nursing.
  • Minimum 3 years of relevant nursing experience.
  • Competent to independently provide nursing care according to established protocols.
  • Strong communication and interpersonal skills.
  • Resourceful and systematic approach to work.
