Job Description

We are seeking a compassionate and proactive Homecare Professional to join our team in delivering high-quality homecare services to clients. This role is pivotal in coordinating and providing personalized care solutions that meet individual needs.

About the Role

You will work closely with clients and healthcare professionals to develop and implement individualized care plans, ensuring alignment with client expectations.

  • Conduct initial assessments of clients' health, living conditions, and homecare needs.
  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare professionals.
  • Coordinate and monitor the delivery of homecare services, ensuring alignment with care plans and client expectations.
Key Responsibilities
  • Provide assisted living care such as showering, toileting, transfer from wheelchair to bed, feeding etc.
  • Serve as the main point of contact for clients and caregivers, addressing inquiries, feedback, and concerns promptly.
  • Promote awareness of homecare services through outreach and engagement activities.
Operational & Administrative Duties
  • Maintain accurate and up-to-date documentation of client records, care plans, service logs, and incident reports.
  • Ensure compliance with internal policies, industry standards, and regulatory requirements.
Quality Assurance & Improvement
  • Conduct regular follow-up to assess service quality and client satisfaction.
  • Identify areas for improvement and implement corrective actions where necessary.
Qualifications & Requirements
  • GCE 'O'/'N' Level, Diploma in Nursing, Healthcare Management, Social Work, Gerontology, or related field.
  • Prior experience in homecare, healthcare coordination, or eldercare services is preferred.
  • Strong understanding of care planning, case management, and healthcare systems.
  • Good interpersonal, communication, and problem-solving skills.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.
  • Proficient in care management systems.
  • Fluency in English and Mandarin, able to speak simple conversational dialects such as Hokkien, Cantonese is an advantage, especially for client engagement.
  • Caring, patience, and a genuine passion for helping elderly.
