Personal Care Specialist

We are seeking a compassionate and proactive individual to join our team in delivering high-quality homecare services. This role is pivotal in coordinating personalized care solutions that meet individual client needs, ensuring seamless transitions and effective case management.

Key Responsibilities:
  • Conduct comprehensive assessments of clients' health, living conditions, and homecare needs, identifying opportunities for improvement and implementing tailored care plans.
  • Collaborate with clients, families, and healthcare professionals to develop and implement individualized care plans that prioritize client-centered outcomes and exceed expectations.
  • Coordinate homecare services, ensuring alignment with care plans and client preferences, while maintaining open communication channels and addressing concerns promptly.
  • Provide assisted living care, including personal care, bathing, toileting, transfer from wheelchair to bed, feeding, and other activities of daily living, with empathy and compassion.
  • Serve as the primary point of contact for clients and caregivers, addressing inquiries, feedback, and concerns in a timely and professional manner.
Requirements:
  • Prior experience in homecare, healthcare coordination, or eldercare services, with a strong understanding of care planning, case management, and healthcare systems.
  • Diploma in Nursing, Healthcare Management, Social Work, Gerontology, or related field, with a focus on evidence-based practice and continuous quality improvement.
  • Strong interpersonal, communication, and problem-solving skills, with the ability to work effectively in a fast-paced environment and adapt to changing circumstances.
Qualifications:
  • Fluency in English, with the ability to communicate complex information in a clear and concise manner, and proficiency in care management systems.
  • Caring, patience, and a genuine passion for helping elderly individuals, with a commitment to dignity, respect, and person-centered care.
