Job Title: Assistant Manager, Senior Social Worker/Senior Case Manager
About the Role
We are seeking a highly skilled and experienced Assistant Manager, Senior Social Worker/Senior Case Manager to join our team at NTUC Health. As a key member of our community care team, you will be responsible for providing high-quality care coordination and support services to our clients and caregivers.
Key Responsibilities
- Develop and implement care plans that meet the unique needs of our clients and caregivers.
- Provide supervision and guidance to our social work team to ensure they are equipped to provide excellent care and support services.
- Collaborate with community partners and stakeholders to build strong relationships and ensure seamless care coordination.
- Monitor and evaluate the effectiveness of our care services and make recommendations for improvement.
- Ensure that our care services are delivered in accordance with relevant laws and regulations.
Requirements
- Bachelor's Degree in Social Work, Psychology, Sociology, Gerontology, or Counselling.
- Minimum 3 years' experience in community healthcare settings in Singapore.
- Good knowledge of geriatrics and community services.
- Proficiency in Google Suite applications.
- Excellent communication and interpersonal skills.
- Passion for community services and service-oriented approach.
