Job Title: Doctor (FMC & Nursing Home)

As a Doctor, you will be responsible for the care of clients seen at the Family Medicine Clinic (FMC) and Residential Care services under NTUC Health. You will work in a multidisciplinary environment with care professionals, including care support staff, nurses, and allied health professionals. Your primary base will be at the FMC, where you will perform scheduled medical rounds and attend to residents in a Nursing Home on some days.
Key Responsibilities:
- Practice at all times in accordance with the SMC Physician's pledge and abide by the SMC Ethical Code of Conduct.
- Stay up-to-date with medical information and developments through Continuing Medical Education (CME) events.
- Stay informed about the group's medical practice.
- Conduct reviews for chronic medical conditions and practice in a manner aligned with the patient's goals of care, according to best medical evidence and practices.
- Diagnose and treat acute medical conditions, prescribe medications, perform minor surgical procedures, and initiate referrals for specialist reviews where necessary.
- Work with a team of fellow doctors, nurses, and care coordinators to provide the best possible outcomes for our patients.
- Support NTUC Health's overarching strategic goals by engaging with community partners, grassroots organizations, and regional health systems where opportunities arise.
- Explore and take part in medical innovation and novel care delivery models.
Responsibilities at Family Medicine Clinic:
- Conduct reviews for chronic medical conditions and practice in a manner aligned with the patient's goals of care, according to best medical evidence and practices.
- Diagnose and treat acute medical conditions, prescribe medications, perform minor surgical procedures, and initiate referrals for specialist reviews where necessary.
- Work with a team of fellow doctors, nurses, and care coordinators to provide the best possible outcomes for our patients.
- Support NTUC Health's overarching strategic goals by engaging with community partners, grassroots organizations, and regional health systems where opportunities arise.
- Explore and take part in medical innovation and novel care delivery models.
Responsibilities at Nursing Homes:
- Conduct medical rounds to review nursing home residents on a regular scheduled basis to attend to their chronic medical needs and acute conditions in a manner aligned with the patient's goals of care, according to best medical evidence and practices.
- Write medical reports, memos, referrals, and administrative forms where required for purposes of application to schemes beneficial to residents and/or to correspond with other healthcare and non-healthcare services.
- Practice with a patient-centered approach, and engage with the resident or next of kin where necessary in order to develop a care plan aligned with the residents' values and desired outcomes.
- Lead a multidisciplinary team of nurses, allied health professionals, social workers, and welfare staff with regular multidisciplinary meetings in order to achieve patient-centered outcomes.
- Guide care staff with regards to best medical practices.
- Support NTUC Health's overarching strategic goals by engaging with community partners, grassroots organizations, and regional health systems where opportunities arise.
- Explore and take part in medical innovation and novel care delivery models.
Qualifications:
- Basic medical degree recognized by the Singapore Medical Council.
- Full registration with the Singapore Medical Council.
- Graduate Diploma in Family Medicine / MMed (Family Medicine) preferred.
- Good communication and interpersonal skills.
- Willingness to serve and connect with the elderly and underprivileged.
- Interest in medical innovation and novel care delivery models.
- Relevant Graduate diplomas in Geriatric Medicine, Palliative Medicine, or Mental Health.
- Experience working with elderly or palliative patients.
- Experience with medical innovation and/or novel care delivery models.
- Proficiency in Google Workspace tools would be advantageous.
