Job Title: Case Manager

At Singapore Anglican Community Services, we are seeking a skilled Case Manager to join our team. As a Case Manager, you will play a vital role in providing person-centered case management services to vulnerable elderly living in the community.

Key Responsibilities:
  • Conduct intake assessments and develop individualized care plans to promote clients' wellbeing.
  • Provide home visits, basic counselling, care coordination, and service linkage to support clients' needs.
  • Work with volunteers to provide befriending and support to clients.
  • Develop caregiver support networks within the specified service boundary.
  • Manage case documentation, home visit logs, and case statistics in a timely and effective manner.
  • Develop and implement appropriate intervention strategies in consultation with the supervisor.
  • Participate in case reviews, case supervisions, and case conferences.
  • Assist in outreach initiatives such as community mental health screening and promoting centre's services to create public awareness of healthy aging.
