Job Title: Executive, Case Management
About the Role:
We are seeking a highly skilled and compassionate Case Manager to join our team at NTUC Health. As a Case Manager, you will play a vital role in ensuring the continuity of care for our residents, working closely with multidisciplinary health service professionals to develop care programs for timely treatment or recovery.
Key Responsibilities:
- Monitor and manage bed occupancy rates, meeting admission KPIs on a monthly basis
- Track and manage residents' data and information, ensuring proper filing and confidentiality
- Apply financial schemes and monitor payouts for residents on a monthly basis
- Renew residents' Household Means-Test before expiration and prepare Medifund reports and documents
- Provide excellent customer service, addressing queries and concerns from residents and their next-of-kin
- Perform other case management tasks as assigned by management
Requirements:
- Diploma in Nursing, Social Work, or related disciplines
- Minimum 1 year experience in community healthcare settings in Singapore
- Good knowledge of geriatrics and community services
- Strong communication and interpersonal skills
- Proficiency in G Suite applications
