About the Role">

As a seasoned professional, you will play a pivotal role in driving the strategic development of portfolios to advance social-health integration and enhance the delivery of care in the community.

This includes ideation and management of collaborations with partners and stakeholders to support the roll out of new policies and initiatives that augment our organization's role as a population health manager for the region.

">

You will be responsible for developing strategies and workplans to augment our organization's role in new health/social policies and to advance social-health integration. This will involve identifying opportunities and holding regular engagements with agency/community partners and internal stakeholders to drive shared successes.

In addition, you will oversee end-to-end project management of assigned collaboration/initiatives, keeping abreast of developments within the health and social landscape. You will also provide secretariat duties to drive progress and conversations in assigned portfolios.
