Key Responsibilities:- Health Promotion & Illness Prevention: Educate patients on making informed lifestyle choices to prevent diseases.
- Primary Care Services: Conduct comprehensive assessments and provide tailored health education to the general public.
- Home Support: Offer vital support to patients transitioning from hospital to home care settings.
- Additional community-related duties as assigned by management.
- Bachelor's Degree in Nursing and current registration with the Singapore Nursing Board.
- A minimum of 2-3 years' experience in a community-based setting.
- The ability to work independently and collaboratively within a team environment.
- Strong organizational, coordination, interpersonal, and communication skills are essential.
- Candidates with conversational proficiency in local languages are highly desirable.
