Job Description:

The Community Health Coach plays a vital role in supporting and empowering residents through personalized health coaching. This involves working closely with residents to develop and implement tailored health plans, as well as facilitating their participation in health-related programs organized by community partners.

Responsibilities include performing health and social screenings to identify areas of need, developing and maintaining accurate records of resident progress, and collaborating with community nurses and managers to support residents in achieving their health goals.

Key aspects of this role include providing basic patient and family education for wellness and disease prevention, conducting preventive health screenings, and participating in health promotion events and activities.

The ideal candidate will possess strong communication and interpersonal skills, particularly when engaging with older adults. Proficiency in computer skills and Microsoft Office is also essential, as is the ability to work independently and as part of a team.

Required Skills and Qualifications:
  • Diploma or advanced diploma qualification in a relevant healthcare field
  • Working experience in the healthcare industry (an advantage)
  • Good knowledge of community services and support in a community setting (an advantage)
  • Strong organizational, coordination, and interpersonal skills
  • Proficient in computer skills and Microsoft Office
