Care Corner Singapore Ltd is seeking a Community Mental Health Coordinator to join our Community Resource, Engagement & Support Team (CREST). This role will focus on raising public awareness of mental health conditions and dementia, early recognition of at-risk individuals, providing emotional support to individuals and their caregivers, and linking them to relevant health and social care services.
Responsibilities- Attend to enquiries and provide information and referral services;
- Support in coordinating and implementing mental health programmes with community stakeholders;
- Engage in public mental health education, engagement/preventive activities, and community outreach initiatives;
- Maintain proper and up-to-date case documentation for case management services under the guidance of a social worker;
- Analyse community needs and contribute to service development to meet those needs;
- Support resource mobilization efforts to strengthen service delivery and care transition for clients;
- Ensure accurate upkeep of programme data, records, and documentation.
The ideal candidate will possess relevant qualifications such as an ITE or Diploma in Community Care and Social Service, Gerontology, Social Sciences, or Psychology. Strong interpersonal communication skills, organisational abilities, and a passion for working in the mental health or eldercare sector are also essential.
