Job Overview:

We are seeking a skilled Nurse to join our team as a Staff Nurse in the Home Healthcare Team. This is an excellent opportunity for you to provide personalized care and support services to clients in their homes.


Responsibilities:
  • Conduct thorough assessments of potential clients and their home environments to identify their needs and develop effective care plans.
  • Work closely with caregivers and family members to create individualized care plans (ICPs) that cater to each client's unique requirements.
  • Regularly review and update ICPs to ensure they remain relevant and effective in supporting clients' well-being.
  • Provide guidance on government subsidies and financial planning to help clients access necessary resources.
  • Coordinate hotline operations and assist doctors during home visits to deliver seamless care services.
  • Develop discharge plans for clients who no longer require our services, ensuring a smooth transition back to their communities.
  • Design and conduct health education and training sessions for seniors and caregivers to promote healthy living and independence.

Requirements:
  • Degree in Nursing or equivalent qualification.
  • Registration with the Singapore Nursing Board.
  • A minimum of 3 years of experience working in a related field.
