Job Overview

As a Clinical Instructor, you will collaborate with multidisciplinary teams to deliver patient-centered care according to assessed needs and health conditions.

You will facilitate timely admissions, transfers, and discharges, ensuring effective discharge planning for each patient.

Monitoring patients' progress, evaluating their response to treatment, and collaborating with the medical team to achieve desired care outcomes are key responsibilities.

Early recognition of clinical deterioration, escalation as required, and timely interventions to ensure patient safety are crucial duties.

Active participation in multidisciplinary rounds, execution of physicians' treatment and investigation orders promptly, and communication of patient progress with the medical team are essential roles.

Clinical skills, knowledge, critical thinking, and reasoning in patient care delivery are vital. You will also assist the nursing team in problem-solving and seek assistance when necessary.

Patient and family education on disease progression, prevention, and rehabilitation, assessing understanding, and enabling self-healthcare management are critical responsibilities.

Reviewing infection control practices, monitoring compliance, and identifying areas for improvement are ongoing tasks.

Planning and implementing in-services for nurses to extend their skills and knowledge, maintaining safe and quality nursing care, are key objectives.

Supporting new initiates, leading changes implemented by Nursing, and reviewing clinical performance of new nurses are additional responsibilities.

Providing constructive feedback and documentation to appropriate personnel and acting as a competent clinical resource are essential roles.

This position requires a minimum degree in Nursing with at least 5 years of relevant permanent inpatient experience and senior nursing experience, preferably in gerontology/palliative care, along with an Advanced/Specialist Diploma in Geriatric/Community Nursing & Long-Term Care settings.
