Community Health Development Specialist

As a member of our team, you will be involved in the development of healthy precincts in collaboration with key stakeholders and partners. Healthy precinct is a geographical location that supports residents to achieve and sustain healthy lifestyles.
The successful candidate will be placed in an appropriate position based on experience.
Key Responsibilities- Support in planning and operationalising health precincts concept to achieve health-social integration with internal and community stakeholders.
- Develop and implement a community health plan for the precincts under your care, with emphasis on preventive health and social-health integration.
- Track and review progress on the implementation of the community health plan for different focus areas.
- Engage & build partnerships with internal and community stakeholders such as grassroots organisations, social service agencies, and healthcare providers.
- Bachelor's degree in any discipline.
- Minimum 2 years of working experience.
- Able to think critically.
- Strong planning, project management, and organisational skills.
- Adept in stakeholder engagement to foster buy-in and collaboration.
- Strong written and verbal communication abilities.
- Resourcefulness in problem-solving and resilience in overcoming challenges.
