We are seeking a skilled and compassionate Disability Care Specialist to join our team. As a Disability Care Specialist, you will play a vital role in empowering individuals with disabilities to live independently and fully participate in their communities.

About the Role:

As a Disability Care Specialist, you will be responsible for assessing client needs, developing and implementing individualized therapy programs, and collaborating with multidisciplinary teams to promote independence and community integration.

Key Responsibilities:
  • Needs Assessment: Conduct thorough assessments of clients' physical, emotional, and social needs to identify areas for intervention and develop effective plans to support their growth and development.
  • Therapy Programs: Design and implement individualized therapy programs that address clients' specific needs and goals, using evidence-based practices and approaches.
  • Collaboration and Communication: Work closely with multidisciplinary teams, including medical professionals, therapists, and caregivers, to ensure seamless coordination of care and effective communication with clients and families.
  • Client Support: Provide ongoing support and guidance to clients and their families, empowering them to make informed decisions about their care and promoting self-advocacy and self-determination.
Requirements:
  • Qualifications: Full registration with the Allied Health Professions Council in Singapore, preferably with experience working in adult disability settings.
  • Skills and Attributes: Strong communication and interpersonal skills, with the ability to build rapport with clients, families, and colleagues; flexibility and adaptability in a fast-paced environment; and a commitment to continuous learning and professional development.
