Job Opportunity

To excel in this role, you will play a pivotal part in fostering partnerships and managing collaborations between healthcare providers.

Key Responsibilities:
  • Community Engagement: Regular engagement with key account community partners to enhance care integration and delivery of care in the community.
  • Feedback Management: Effective management of feedback and issues with/from community care partners.
  • Staying Current: Keeping abreast of developments within long-term care and community care organisations.
  • Strategic Collaborations: Identifying opportunities for strategic collaborations with key partners to improve care integration and augment community care.
  • Collaboration Management: Management of collaborations/initiatives/projects to improve care integrations between healthcare providers and/or augment community care.
  • Secretariat Services: Serving as workgroup/meeting secretariat.
  • Population Health Strategy Implementation: Supporting implementation of population health strategies and regional health plans.
Requirements:
  • A good degree in Life Sciences, Business Administration, or a related field is preferred.
  • Three to five years of relevant working experience, preferably in community care or partnerships.
  • Experience in project management and working with multiple stakeholders is essential.
  • Confidence to engage internal and external stakeholders at all levels is required.
  • Ability to multitask and work under tight timelines is necessary.
  • Good analytical and organisational skills are essential.
  • Excellent written and verbal communication, problem-solving, presentation, negotiation, and interpersonal skills are required.
