Job Overview

The Assistant Manager will play a pivotal role in driving collaboration between Intermediate & Long Term Care (ILTC) providers and healthcare organizations to enhance care integration and delivery.

Key Responsibilities
  • Engage regularly with key community partners to improve care coordination and service delivery
  • Manage feedback and issues with/from community care partners effectively
  • Stay abreast of developments within ILTC and community care sectors
  • Identify opportunities for strategic collaborations with key partners to enhance care integration and augment community services
  • Oversee the management of initiatives/projects to improve care integrations between healthcare providers and ILTC organizations
  • Provide secretariat support for workgroups/meetings
  • Support the implementation of Population Health strategies and regional health plans
Requirements
  • Good Class Degree, preferably in Life Sciences, Business Administration or related field
  • 3 to 5 years of relevant working experience, prior experience in community care or partnerships an asset
  • Proven project management skills and ability to work with multiple stakeholders
  • Confidence to engage internal and external stakeholders at all levels
  • Able to multi-task and work under tight timelines
  • Excellent analytical, organization, written and verbal communication, problem-solving, presentation, negotiation and interpersonal skills
