Job Title: Senior Medication Specialist

About the Role:

You will be part of a multidisciplinary team responsible for maintaining current health and promoting wellness in patients. Your daily tasks will involve identifying, resolving, and preventing drug-related problems, recommending and monitoring patient-specific drug therapy, and ensuring safe and effective alternatives are available.

Key Responsibilities:

  1. Identify, resolve, and prevent drug-related problems;
  2. Recommend and monitor patient-specific drug therapy;
  3. Document outcomes and oversee drug distribution;
  4. Actively participate in regular inpatient clinical rounds and/or ambulatory care clinics;
  5. Dispense medications and counsel patients on their medication treatment;
  6. Communicate timely and accurate drug information to patients, the public, and healthcare professionals;
  7. Provide education on safe and effective medication use to patients, students, and fellow healthcare workers;
  8. Contribute to research efforts in clinical and/or translational areas to advance knowledge in the specialty;

Requirements:

  • Eligibility for registration with the Singapore Pharmacy Council;
