Medical Social Support Specialist

We are seeking a highly skilled and compassionate Medical Social Worker Assistant to join our team. In this role, you will play a vital part in supporting clients and their families by exploring possible interventions to reduce their social and healthcare costs.
- Liaise with clients and families to follow up on pending assistance, including Medifund, Household Means Testing (Subvention), and other funding schemes.
- Stay updated on available financial assistance and provide advice to clients and their families on eligible resources.
- Support and adhere to care plans developed by Centre Managers and other healthcare colleagues.
- Monitor clients' social wellness and participation rate in social activities during their stay.
- Assist in follow-up and liaison with community services for discharge care plans, if necessary.
- Provide financial counselling for all clients during assessment.
- Closely monitor referral intake KPI of 10 days.
- Perform administrative tasks, such as keeping records, filing, and updating progress reports.
- Diploma in Social Work.
- Fresh graduates are also considered.
- At least 1 year of relevant healthcare industry experience.
- Pleasant and mature disposition with empathy and patience in liaisons with everyone.
- Demonstrate initiative, good communication, and people engagement skills.
- Linguistic capability in English, Mandarin, and advantageous use of local dialects to interact with local clients.
- Good organisation ability.
