We are seeking a dedicated and compassionate professional to join our team as a Community Care Specialist.
The ideal candidate will have a strong background in nursing, mental health, or a related field, with experience working in community settings. You will be responsible for providing high-quality care coordination, psycho-education, and support to clients and their families.
- Conduct home visits to assess patients' medical, psychological, social, environmental, and vocational needs.

Requirements:
- Registered Nurse with the Singapore Nursing Board.

- 3 to 5 years of psychiatric experience (Community psychiatric clinic preferred).
- Skilled in psychotherapy for mental health patients.
- Strong organizational, interpersonal, communication, and writing skills.
