As a member of our multidisciplinary team, you will provide holistic and comprehensive care to patients. This involves empowering individuals and their caregivers to take charge of their own health through the use of technology.
Strategic objectives include building relationships with internal stakeholders (care team) and external stakeholders (patients and caregivers). Key outcomes include achieving clinical results for preventive health screenings and reducing readmissions and escalations to tertiary care. Additionally, gathering social determinants of health that positively influence social and health outcomes is also essential.
- Perform basic parameter measurements and close preventive screening and vaccination gaps in relation to national initiatives.
- Provide basic health education to improve patients' knowledge on lifestyle management, social and physical activities through social prescribing. Advise patients on vaccination requirements and monitor health metrics using devices like home blood pressure, glucose, and weight monitoring.
- Diploma preferred. Qualification in health promotion may be advantageous.
- Previous working experience in healthcare is beneficial.
- Good verbal and written communication skills are necessary.
- Pleasant disposition and personality are required.
- Excellent interpersonal skills, positive attitude towards work, and teamwork capabilities are essential.
- Effective communication and coordination skills are necessary.
- Self-motivation and interest in patient care are required.
- Able to communicate in English and local dialect.
- Proficient in Microsoft Office including Excel, Words, and PowerPoint.
- Skilled in navigating IT programs.
