Comprehensive Care Specialist

As a Comprehensive Care Specialist, you will provide high-quality nursing care to patients in their own home setting. Reporting to the Associate Director and Nurse Manager, your responsibilities include:
- Executing basic to complex nursing procedures, including medication administration, vital signs monitoring, ECG monitoring, pain management, wound dressing, vaccinations, blood glucose monitoring, etc.
- Conducting patient assessment, providing appropriate nursing intervention, and evaluating the outcome of intervention, escalating required situations to doctors.
- Effectively communicating patient's condition to the internal and external healthcare team and providing accurate updates via documentation on the Client Management System.
- Actively participating in handover meetings with partner hospitals to ensure seamless continuity of care.
- Providing caregiver training for patients and family members as required.
- Ensuring safe and high standards of care provision to meet clients' expectations.
The ideal candidate will have a passion for inpatient and community nursing, and be able to thrive in a dynamic environment. They should be a team player with good initiative, strong client focus, and ability to work under high pressure.
