We raise public awareness of mental health conditions and dementia, promote early recognition of at-risk individuals, provide emotional support to individuals and their caregivers, and link them to relevant health and social care services when necessary.

  • Coordinate care and support to clients with or who are at risk of mental health issues and/or dementia with appropriate resources.">
  • Assess and provide intervention to clients, including referring cases to other agencies as needed.">
  • Provide emotional support to clients and caregivers and maintain case documentation.">
  • Outreach and networking to the community and stakeholders to promote mental wellness and mental health awareness.">
  • Plan and conduct cognitive stimulation activities, mental health/wellness programs as well as dementia awareness.">
  • Organize caregiver group sessions and manage administration and finances related to client services.">
  • Maintain accurate client records and program data in line with organizational guidelines as well as adherence to privacy and security standards.
