9. Perform general clinical pharmacy services to promote wellness and ensure optimization of medication therapy outcomes for patients.
10. Identify and define patient-specific medical problems, recommend, implement, and manage patient pharmacotherapy.
11. Monitor and adjust patient drug therapy, resolve and/or prevent patient drug-related problems.
12. Promote and monitor adherence to prescribing guidelines or drug use protocols.

Requirements:
Possess MSc (Clinical Pharmacy) or Pharm D or Board Certification.
Completion of a Collaborative Prescribing Course within last 2 years.
A minimum of 5 years of clinical experience (ideally in inpatient care).
Registered with the Singapore Pharmacy Council.
Possess extensive and current knowledge of pharmaceuticals, clinical practice guidelines, and protocols.
Possess strong leadership qualities, good supervisory, negotiation, and managerial skills.
Excellent clinical pharmacy and evidence-based medicine knowledge and skills.
Excellent clinical problem-solving, judgment, and decision-making skills.
Excellent knowledge of drug information research tools, clinical research methodologies, and outcomes evaluation.
Excellent listening and communication skills, with ability to educate and engage patients, and work collaboratively with diverse healthcare team.
