Job Title:
Senior Clinical Pharmacist

Overview
This role involves leading and supervising a team of pharmacists in the provision of patient care services. The Senior Clinical Pharmacist will be responsible for reviewing medication charts, ensuring that orders are clear and specific, and identifying potential adverse reactions and interactions.

Key Responsibilities
  • Conduct clinical reviews for the appropriateness of medication orders.
  • Review each patient's medication chart or prescription for the appropriateness of the drugs prescribed and contact the prescribing doctor when an intervention is needed.
  • Ensure operational efficiency to meet desired turnaround times.
  • Supervise performance and conduct appraisals.
  • Involve in national or institutional workgroups and projects.

Requirements
Education / Training / Experience
Bachelor of Science degree in Pharmacy from an accredited University recognized by the relevant authorities. Registered with the relevant authority.

Attributes
Clinical knowledge for review and counselling. Relevant skills to conduct root causes analysis. Learning attitude; willing to learn from junior staff when applicable.
