Cancer Navigator Position

Job Title: Cancer Support Specialist
Role Overview: The Cancer Support Specialist provides personalized care and support to adult cancer patients, enhancing their overall well-being and quality of life.

Key Responsibilities:

  • Support clients with diverse needs, from newly diagnosed to palliative/end-of-life care and survivorship.
  • Offer psycho-social and emotional support for clients and their caregivers.
  • Connect clients with relevant resources and services, such as financial assistance, food, transportation, mobility equipment, and medical consumables.
  • Collaborate with community partners to ensure continuity of care after referrals.
  • Refer clients for counseling, spiritual or mental health support when needed.
  • Provide information and coordinate education from healthcare practitioners to improve understanding of cancer diagnoses and treatments.
  • Maintain relationships with clients and caregivers to assess coping during the cancer journey.
  • Advocate for cancer patients and promote awareness through community programs.
  • Document case recordings and facilitate support group sessions.

Requirements:

  • BA/BS in Social Work, Psychology, or related field.
  • Minimum 2 years of experience working with adult cancer patients.
