Job Title: Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team.

About the Role:
  • This is a challenging yet rewarding role that requires excellent communication, interpersonal and problem-solving skills.
  • You will be responsible for conducting thorough assessments, formulating individualized care plans and providing interventions for individuals with dementia and their caregivers.
Key Responsibilities:
  • Conduct case assessments, develop comprehensive care plans and provide supportive counseling as needed.
  • Liaise with multidisciplinary professionals and external service providers to ensure holistic care management.
  • Document case records and relevant information.
  • Participate in team meetings and case conferences as required.
  • Collect and analyze data for program evaluation and reporting purposes.
Requirements:
  • Prior experience in dementia care is preferred but not essential.
  • Excellent oral, written and interpersonal communication skills.
  • A degree in an allied health profession, social work or psychology is desirable.
  • Flexibility to work occasional Saturdays and weekday evenings is required.
