Community Health Navigator


Job Description:
  • Engage and Perform Screening: Engage with children and family members identified by or referred to our organization, performing relevant screening processes.
  • Health Information Provision: Provide basic and personalized health information to clients and families.
  • Social-Health Navigation: Offer support to clients and families in navigating the social-health system by providing information and resources tailored to their needs.
  • Collaboration and Referrals: Collaborate with external agencies and community partners on referrals for timely follow-up.
  • Program Promotion: Publicize mother-child dyad and parenting programs to children and families.
  • Service Identification and Referral: Identify clients and families in need of subsidized services and facilitate referral to financial, social, and health services.
  • Documentation: Perform accurate documentation in client records during the delivery of care.
  • Health Promotion Activities: Support group-based health promotion activities conducted both in clinics and communities, together with institution partners.
  • Engagement Support: Provide support for engagement activities at clinics with supervisor approval.
  • Defaulted Referrals: Track, recall, and reschedule referrals for clients who defaulted on children and women's health-related services and follow up according to established workflows.
  • Care Coordination: Coordinate care for clients and families with the team in Women and Children's Services.
  • Patient Safety and Quality Improvement: Adhere to the International Patient Safety Goals (IPSG) and infection control practices. Maintain a clean and safe working environment and report unsafe practices and incidents according to quality standards.
  • Professional Development: Attend training courses to acquire new knowledge and skills, assess own learning needs, and undertake learning as appropriate.
  • Administrative Tasks: Follow-up on referrals to and from our organization, assist in tracking clinical indicators set for Family Nexus, conduct surveys gathered from users' feedbacks, and assist in planning and supporting group activities conducted in clinics or communities.

Secondary Responsibilities:
  • Organizational Projects: Participate and support organizational projects and events.
  • Planning and Coordination: Assist in planning, organizing, and coordinating activities with the supervisor.
  • Housekeeping Duties: Provide incidental housekeeping duties including maintaining cleanliness of workplace and topping up stationery and other consumables.
  • Additional Tasks: Perform any other duties assigned by the supervisor.
