Job Title: Home Care Coordinator
We are seeking an experienced and compassionate professional to provide home nursing services to clients and support their caregivers. The ideal candidate will collaborate with internal and external stakeholders to facilitate effective care and enable clients to age in place.
Key Responsibilities:
- Conduct home visits for assigned clients to assess and develop a comprehensive nursing intervention and care plan.
- Perform nursing procedures in the home environment, utilizing available resources, and implementing necessary safety measures.
- Provide patient education to promote improved health outcomes.
- Liaise with healthcare partners for efficient and effective care planning.
- Train caregivers on best practices.
- Coordinate discharge processes upon client admission.
Additional Duties:
- Screen referrals from external agencies.
- Participate in new initiatives and projects as assigned.
- Share knowledge on care strategies for complex medical/social/mental cases.
- Complete initial assessments for Home Personal Care (HPC) clients in collaboration with HPC supervisors.
