Job Description
We are seeking a qualified Clinical Nurse to join our team. The successful candidate will be responsible for conducting home visits, performing nursing procedures, and developing patient care plans. You will work closely with the medical team to ensure high-quality patient care and respond promptly to clinical conditions.
Key Responsibilities
- Conduct home visits and nursing assessments to identify patient needs and develop personalized care plans.
- Perform nursing procedures and assessments as required.
- Triage clinical conditions and respond promptly to ensure patient safety.
- Develop and implement patient care plans in collaboration with the medical team.
- Maintain accurate records of patient progress and treatment outcomes.
Requirements
- Candidates must have a Diploma or degree in Nursing and at least 1-2 years of experience working in acute hospitals.
- Registered with the Singapore Nursing Board is a requirement.
- Prior experience with dialysis machine operation is an advantage but not necessary.
