Job Title

The purpose of this role is to support individuals going through transitions in their lives.


Key Responsibilities
  • Work with clients and their families to develop and implement individualised plans that meet their needs and goals.

This involves working closely with a range of stakeholders, including healthcare professionals and social workers, to ensure that clients receive the best possible support and guidance.


  • Assess client's transition and development needs regularly, to track progress and make any necessary adjustments to their plan.
  • Document and keep records of outcomes of clients' Individual Development Plans (IDPs) and produce consolidated reports on overall IDP outcomes.

Clients' Training
  • Carry out training and support sessions for clients' transition training needs at relevant settings.
  • Document and keep records of clients' training progress and adaptations made.

Collaboration and Advocacy
  • Work with relevant stakeholders to advocate for clients' transitional and developmental needs according to their IDP, looking at environmental, social emotional, behavioural support factors.
  • Document and keep records of engagements, adaptations made with relevant stakeholders.

Requirements
  • Minimum Diploma in Social Work, Psychology, Community Services or other relevant fields with at least 2 years working experience.

  • A strong team-player with excellent people skills.
  • Possesses strong organisation skills, time management and problem-solving skills.
  • Possesses strong written and verbal communication skills.
  • Ability to speak one additional local language apart from English an advantage.
  • A self-motivated and adaptable individual.
  • Ability to work well both independently & in a team with integrity, sensitivity, knowledge and drive to support & advocate for their clients with intellectual disability.
  • Passion to work with people with intellectual disabilities.
