Job Title: Transition Support Specialist

About the Role:

We are seeking a skilled Transition Support Specialist to join our team. This role will involve working closely with individuals with disabilities, employers, and support networks to facilitate their transition from education to employment.

Main Responsibilities:
  1. Client Servicing:
  • Work with clients, employers, and support networks to facilitate internship or job placement.
  • Provide job support to clients at their workplace.
  • Collaborate with various teams to provide support to employers in their disability inclusion journey.
Stakeholder Management:
  • Engage with external stakeholders (e.g., employers and Social Service Agency partners) to support the employment of individuals with disabilities.
  • Manage and work closely with appointed programme partners on client referrals, service model, track and report progress.
Evaluation and Development:
  • Analyze data to evaluate programme effectiveness, identify gaps, and develop strategies to enhance employability.
  • Develop and execute strategies on outreach and engagement of individuals with disabilities in educational institutions, in partnership with Special Educational Needs and/or Education & Career Guidance offices.
Requirements:
  • Degree holder in any discipline, with 2-3 years of work experience.
  • Experience in the social service/disability sector or client-facing roles would be an advantage.
  • Team player with excellent communication, interpersonal, and analytical skills.
  • Flexible and adaptable to changing circumstances and versatile to take on other roles when required.
  • Strong time management with the ability to work under tight deadlines and fast-paced environment.
  • Meticulous in administrative tasks and resourceful in problem-solving.
  • Comfortable with public speaking and stakeholder engagement.
