
Care Coordinator

The role of a Care Coach involves supporting the care team to enhance patient care coordination across the continuum. This position requires collaboration with multi-disciplinary team members to provide comprehensive care and advocate for patients' and families' complex psychosocial needs.
Key responsibilities include:
- Screening patients for their bio-psycho-social, environmental, vocational, and financial needs.
- Interviewing patients and families to understand their health beliefs and attitudes that influence their views on health and health-seeking behaviors.
- Gathering information about patients' and families' social support systems and socioeconomic resources that impact their health and care.
- Synthesizing gathered information to identify predisposing factors of patients' bio-psychosocial needs and prioritizing care needs with the care team.
- Working with the care team to plan and implement patients' care plans, including lifestyle and behavioral health modifications, medication management, self-monitoring, and psycho-social support.
In addition, Care Coaches must possess excellent interpersonal, communication, and coordination skills to effectively engage, educate, and empower patients and families in their care.
