About the Role:
  • The successful candidate will be responsible for implementing patient care in collaboration with the multi-disciplinary teams (MDT) and according to assessed needs and health condition(s).
  • Facilitating admissions, transfers and discharges; ensuring discharge planning is done timely upon patient's admission and appropriate to condition.
  • Following up and evaluating patients' progress according to planned care and readiness for discharge.
  • Collaborating with the MDT to deliver evidence-based care to patients to achieve the desired care outcomes.
  • Monitoring patient for changes in condition, recognising early signs of clinical deterioration using appropriate physiological assessments and escalating as required to enable timely implementation of interventions.
  • Participating and contributing actively in MDT rounds, executing physicians' treatment and investigation orders promptly.
  • Preparing and assisting physicians with bedside patient procedures and treatments.
  • Communicating and discussing patients' progress and response to treatment in collaboration with the MDT.
  • Demonstrating clinical skills, knowledge, critical thinking and reasoning in the delivery of patient care.
  • Assisting nursing team to problem solve if there are any patient/clinical/or operational issues and seeking appropriate assistance/escalating to Senior Nurse Manager (SNM)/Nurse Manager (NM).
  • Delivering patient and/or family education on disease progression, prevention and rehabilitation and assessing understanding of condition to enable participation in self-health care management.
  • Supporting new initiatives, leading changes implemented by Nursing in collaboration with other stakeholders.
  • Reviewing clinical performance of new nurses and nursing students and providing constructive feedback and documentation to appropriate personnel.
Requirements:
  • Minimum Degree in Nursing with minimum 5 years of relevant permanent inpatient & senior nursing experience, preferably in gerontology/palliative care with Advanced/Specialist Diploma in Geriatric/Community Nursing & Long-Term Care settings.
