Job Title: Advanced Practice Nurse

We are seeking an experienced and skilled Advanced Practice Nurse to join our team.

  • Evaluate, develop, implement, and monitor individualized care plans for residents with complex medical needs in accordance with current nursing protocols and standards.
  • Provide education and support to residents as well as their families or caregivers to promote understanding and involvement in care.
  • Coordinate discharge planning efforts for residents who are deemed suitable for discharge.
  • Ensure that nursing services are provided in line with recognized clinical standards and evidence-based practices.
  • Maintain up-to-date nursing policies and standard operating procedures (SOPs) to ensure consistent, high-quality care delivery.

Requirements:

  • More than 3 years of relevant working experience.
  • SNB Registered.
