Job Title: Professional Home Health Care Specialist

Job Overview:

We are seeking a skilled and compassionate professional to join our team in delivering high-quality home health care services.

Main Responsibilities:

  • To participate in a pilot home palliative project, providing hands-on bedside care to patients with dignity and compassion.
  • Take on respite assignments as needed, ensuring seamless continuity of care for our clients.
  • To collaborate with the home health care team to deliver comprehensive nursing services to clients in their homes or hospitals.
  • Offer guidance and support to care staff, addressing medical issues or emergencies that may arise during visits.
  • Conduct regular hospital or home visits to assess new clients for home health care services or provide ongoing care.
  • Work together with the team to manage complaints and crisis situations effectively, maintaining open communication with clients and caregivers.
  • Maintain strong relationships with external community partners and colleagues to ensure optimal client care and referrals.
  • Ensure accurate and timely documentation, adhering to regulatory guidelines for quality audits and compliance.
  • Perform additional tasks as assigned by management to meet the evolving needs of our clients.

Requirements:

  • A diploma or bachelor's degree in nursing is required for this position.
  • Registered Nurse with a clean disciplinary record, holding current registration with the Singapore Nursing Board (conditional licenses not eligible).
  • Proficiency in Microsoft Office and comfort with IT tools is desirable for efficient documentation and reporting.
  • Flexibility for occasional shift work, particularly for palliative patients, is necessary to accommodate varying client needs.
