Community Care Sector Transformation Specialist

The Landscape Manager will play a key role in shaping the community care sector landscape. They will develop practical resources and strengthen capabilities across the community care sector to ensure that care is well-positioned within the community.
Our ideal candidate has strong analytical skills and attention to detail, with effective interpersonal, stakeholder management, and written communication abilities. They must be able to work independently and collaboratively in a team setting.
Key Responsibilities- Work closely with divisions to develop and refine the Integrated Community Care Provider implementation plan for effective execution at the national and local level
- Monitor and review overall progress of ICCP ground implementation, including grant administration, report analysis, and trend identification
- Drive developmental workstreams such as care journey development, sector capability building, IT systems development, sector engagement, and partnerships in relation to ICCP implementation
- Foster collaborative relationships with MOH, internal divisions, and sector partners
- Support stakeholder engagement sessions and secretariat duties for key meeting platforms
- Recognized degree with at least 2 years of relevant experience, preferably in healthcare or community care sector
- Strong analytical skills and attention to detail, with effective interpersonal, stakeholder management, and written communication abilities
- Ability to work independently and collaboratively in a team setting
- Proactive, self-motivated, and adaptable to fast-paced dynamic environments
- Comfortable with ambiguity and demonstrates flexibility in navigating evolving situations
- Good working knowledge of project and programme management, including data reporting and analysis
