Transforming Community Care: A Key Role
This is an exciting opportunity to shape the future of community care in Singapore. As a key member of our team, you will play a crucial part in reorganizing the community care landscape and transforming how care is delivered.
The role involves working closely with stakeholders to develop and refine the Integrated Community Care Provider (ICCP) implementation plan. This includes monitoring progress, reviewing reports, and providing updates to sector partners and senior management.
Key responsibilities include:
- Driving developmental workstreams such as care journey development, sector capability building, IT systems development, and sector engagement.
- Fostering collaborative relationships with MOH, internal divisions, and sector partners.
- Supporting stakeholder engagement sessions and secretariat duties for key meeting platforms.
Requirements:
- A recognized degree with at least 2 years of relevant experience, preferably in healthcare or community care.
- Strong analytical skills, attention to detail, and effective interpersonal, stakeholder management, and written communication abilities.
- Ability to work well independently and collaboratively in a team setting.
- Proactive, self-motivated, and adaptable to fast-paced environments; capable of managing multiple projects concurrently.
- Comfortable with ambiguity and demonstrates flexibility in navigating evolving situations.
- Good working knowledge of project and programme management, including data reporting and analysis.
