Job Title: Patient Care Specialist
We are seeking a skilled Patient Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the growth and development of our organization.

You will be responsible for assessing patients' conditions, developing individualized treatment plans, and implementing evidence-based interventions to achieve optimal outcomes.
Key Responsibilities- Conduct thorough assessments of patients' conditions, including medical history, range of motion, strength, and functional abilities.
- Plan, discuss with patients, develop and implement individualized treatment plans based on above findings and goals set by patients and referring healthcare professionals (if applicable).
- Utilize a variety of treatment techniques, such as manual therapy, therapeutic exercises and modalities on patients so that they will heal symmetrically and improve their function.
- Continuously monitor and evaluate patient progress, adjusting treatment plans as necessary for optimal outcome.
- Provide education and guidance to patients on injury prevention, self-management techniques and home exercise programs.
- Maintain accurate and detailed patient records, including treatment plans and progress notes.
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated patient care.
- Bachelor's Degree/Post Graduate Diploma/Professional Degree in Physical Therapy/Physiotherapy or equivalent from an accredited institution.
- Be registered with AHPC and have and maintain a Practicing Certificate (PC).
- At least 1 Year of working experience in the related field.
